After compiling over 300+ case studies in the Catalog of Over 300 Heavy Civil Construction Case Studies, one thing becomes clear:

The industry does not have a lessons-learned problem.

It struggles to apply lessons learned.

The same failures keep repeating across different projects, teams, and scopes.

Just with different drawings.

– Earthwork embankment failures rarely happen without warning. Small slope movement gets normalized until instability develops.

– Mobile crane incidents often start with a rushed setup, a poorly designed lift plan, or incorrect assumptions about the ground conditions.

– Stormwater drainage failures begin with small signs: ponding, field grading changes, or temporary drainage modifications that never get re-evaluated.

– Falsework failures frequently trace back to field modifications that were never brought to the attention of the design engineer, sequencing deviations, or temporary bracing changes that were never formally reviewed.

– Concrete mix design failures often start with “minor” field water adjustments made for workability that slowly redefine the approved mix.

– Roadway pavement failures commonly stem from subgrade assumptions, stabilization shortcuts, or compromises in lift thickness made to maintain production.

And almost every one of these failures gets amplified by the same thing:

Schedule pressure.

Nobody says:

“Ignore engineering.”

But we do say:
– “We will check it later.”
– “Let’s not stop production.”
– “We are too far along now.”

That is usually how failure actually begins.

Most failures are not due to unknown risks.

If lessons learned do not change:

– field execution

…then they are not really lessons learned.
They are just documentation of repeated failures.
